Before Your Surgery

You will be asked to sign consent forms for your surgical procedure at your doctor’s office. Your physician will schedule your surgery and provide you with a pre-admission testing form to complete. Return the completed form to our Admitting Department as soon as possible.

Several days before your procedure, your doctor may order a series of pre-admission tests and evaluations to be done at the Medical Center. These will take approximately 90 minutes to complete. During this time, a Nurse Practitioner or Care Manager will answer any questions you have concerning anesthesia and surgery.

Call our Pre-Admission Testing Unit (PAT) at (201) 894-3187, Monday through Friday, from 8 am to 5 pm, on Saturday from 7 am to 11 am to schedule your pre-admission testing appointment. The PAT Unit will tell you what forms you need to bring with you.

Please check your insurance coverage to see if “pre-certification” is required before a procedure can be performed. If pre-certification is needed, you, your physician or the Medical Center may be responsible for obtaining it. If you fail to receive pre-certification, your insurance company may refuse to pay for the procedure.
