Independent Physician Notice

IMPORTANT NOTICE 

The physicians providing professional services in the following areas:

are not employees of Englewood Hospital and Medical Center.  They are independent contractors. Also your attending
physician and any other specialist that you are referred to may or may not be an  employee of the Medical Ceter.

Always ask your physicians to identify themselves and to explain if they are employed by the hospital.   
Remember you have the right to choose your physician and to refuse to be treated by any physician.

Please remember that each physician’s arrangements with individual health care plans are  COMPLETELY INDEPENDENT 
OF THE MEDICAL ENTER.  Some physicians may elect to participate in certain plans while some may have been excluded 
due to limits set by the insuranc companies. Your insurance may  provide complete payment of some of these services; 
however, it may not cover all of these services.  IT IS YOUR RESPONSIBILITY TO CHECK WITH YOUR INSURANCE COMPANY 
TO DETERMINE COVERAGE.  

Some plans require Pre‐admission Notification and Approval, Second Opinions for Elective Surgery and Concurrent 
Utilization Review.  Compliance will result in more complete coverage of the patient’s hospital 
and physician bills and minimized the patient’s out‐of‐pocket expenses.
